A lich almost always has two names, and the tension between them is the whole effect. There is the mortal name — the scholar, archmage or priest they used to be — and the name taken after the transformation. Vaelin Orstrath was a person. The Pale Aggregate is what remains. When both appear in a campaign, the moment a player learns the first one is a genuine reveal, because it reframes the monster as someone who chose this.
The mortal name should sound ordinary for your setting’s scholarly class: Aldric Venn, Sethara Muil, Corvin Askegard. Nothing ominous. The horror comes from how unremarkable it is.
Three patterns carry the most weight. The definite-article title strips away personhood entirely: the Hollow Archivist, the Sallow King, the Thirteenth Silence. The corrupted original keeps a recognisable root but decays it — Orstrath becomes Orsthrall, Muil becomes Muilinox. This is the subtlest and often the best, because players hear the echo before they understand it. The claimed epithet is what the lich calls itself in its own writing: Nachtherren, Undying Solt, Kaltvarn the Unwritten. Hard consonants and Germanic or Slavic sound-shapes do a lot of work here.
Resist stacking apostrophes and silent letters. A lich name has to be spoken aloud at a table, often in a whisper, and if the DM stumbles over it the menace evaporates. Aim for something a frightened NPC could gasp in one breath. If the name needs a pronunciation guide, it is decoration rather than a name.
Name the phylactery and the lair with the same instinct. A lich called the Hollow Archivist keeps its soul in the Index, not in a generic amulet — matching the artefact to the epithet makes the whole villain feel authored rather than assembled.
